Liability Waiver

Gopa Kuteeram Program Liability Release and Media Release

Liability Release:

• I understand fully well that the Gopa Kuteeram program being conducted by Global Organization for Divinity (G.O.D.) may include activities that involve stretching, balancing, bending, ground poses (yoga), jumping, running, etc. indoors or outdoors, which could result in an injury to the child. My child(ren) is/are taking part in this program at his or her own risk and I do not hold G.O.D., the Gopa Kuteeram Program, or its members or volunteers responsible for any unfortunate events inside the program venue, on the way, outside the class, in the parking lot, during a field trip, etc.

• I give permission to the program teachers/counselors to seek medical assistance on my behalf for my child(ren) from a certified professional caregiver in the event of a serious accident or illness, and waive any claim against G.O.D. and its members.

• I understand that all the program teachers, counselors and volunteers have been appointed in good faith and considering their experience with teaching children and with no legal background checks. By signing this waiver, I agree to release G.O.D. and its members of all responsibility for any injuries incurred during the program.

Media Release:

I allow G.O.D. to use photos or videos of my child for G.O.D./Gopa Kuteeram public relations purposes in newspapers/magazines/websites and other media/social media outlets.
